**If you are a prospective family or student, please review the Prospective Students & Families page before completing the form.**

I require all students to complete the Annual Enrollment Form once per year. Parents/guardians must verify or provide correct student information and sign the form, indicating that they understand and agree to the current Policies & Expectations.

Please note that completing the Annual Enrollment Form does not guarantee a spot in the studio OR to be waitlisted for a spot in the studio.

Lessons may start at any point during the year. Enrollment, after a Term begins, will be considered at my discretion, or the student will be added to a waitlist.

If lessons are discontinued at any point, a regular spot in the studio may be forfeited and given to a waitlisted student.
